Diagnostic Assessment Tools of Elective, Emergency and Unscheduled Care Patient Flow
Assessment of current care management processes of the elective, emergency and unscheduled care patients. The assessment identifies best practices as well as change opportunities in improving care management efficiencies for patients in facilities and triage processes to community resources and primary care. The results report generated from the Diagnostic Assessment product identifies the:
- Ability of the Acute Trusts to manage the acutely ill patients in an efficient and cost effective manner while providing clinically appropriate and safe care
- Care Management processes that dovetail to the Ambulance and Acute Trust admission assessment processes, minimizing avoidable admissions through efficient and timely triage to community resources
- Use of relevant tools used by staff in A&E, Admissions Assessment Unit such as clinical evidence based admission criteria and by Ward staff such as clinically based discharge criteria
