Case Management Program Development (in-hospital and/or in-community)

This product includes a complete assessment of current staff roles involved in patient care admission assessment, care coordination and discharge planning. Following identification of staff best suited to transition to the role of a Case Manager, a Case Management Academy is hosted for up to 12 days to educate staff to their role as a Case Manager and to clarify through MADC that staff are competent and capable in their role as a Case Manager.

Additional staff involved in patient triage, chronic disease care, discharge planning and social service assessment are invited to be a part of the Academy training program so that role distinction is clarified and understood and prime to their future as a Case Manager.

In-hospital Case Managers are critical to the ability of a PCT/Trust staff compliment to identify and triage ‘avoidable’ admissions from community, A&E, MAU, CDU as well as to coordinate patient care toward an efficient discharge of those patients admitted to Ward. NHS Trusts and PCTs understand the need for trained/certified Case Managers in this new environment of Performance Improvement, Payment by Results and Performance Based Commissioning.
